If a student requires medication during school hours, parents must complete a medication release form (CCF-943). No medication of any kind can be administered without this completed form. This includes all prescription medication AND over the counter drugs such as aspirin, cough drops, and inhalers. The school nurse or first aide safety assistant will be glad to review all procedures with you.

 

Students who become ill or injured at school should report to the office with a health pass from their teacher. If the illness or injury is such that the student should go home, the parent will be notified. It is essential for us to have a current work phone number and an emergency number to use when the parent cannot be contacted.
